New Patient Resources

Welcome to the Pelini Eyecare family! Please take a few minutes to review this information in preparation for your first eye examination in our office.

Before scheduling your appointment:

Please have any vision insurance information at hand. We will need the insurance company name, the name and birth date of the policy owner, the member ID, and the name and birth date of the patient if he/she is not the policy owner.

What to bring to your appointment:

  • A copy of your current prescription if you have it. It’s not required, but we find it helpful.
  • Your current eyeglasses, sunglasses (prescription or non-prescription), and contact lens boxes or packets if you are a contact lens wearer.
  • A photo ID and a copy of your vision and medical insurance card(s) if available. Some plans do not issue insurance cards, and we know this. We’ll let you know if we have any problem finding your coverage.
    • Please note that if you are visiting us for a medical complaint (eye infection, irritation, or injury) we will request your medical insurance information rather than your vision insurance information.
    • Completed copies of the following forms for each patient we are seeing. Having these ready will save you time in the office on the day of your appointment. If you are not able to prepare these in advance please arrive 10 minutes before your appointment so you will have time to complete the forms.
    • Please print out and fill in our Patient Intake Form.
    • Please read our Privacy Policy and then print & sign our Signature Form.
    • Please read, print, and sign our Financial Policy.
  • Form of payment. Payment is due at the time of service. We accept cash, checks, Visa /Mastercard /Discover /AmEx, Flexible Spending, and Health Savings Account funds.

What to expect during your first visit:

Dr. Pelini or Dr. Phu will thoroughly examine the health of your eyes including the internal and external tissues. The doctor will screen for glaucoma by administering numbing drops to your eyes and then use a tonometry pen to measure your intraocular pressures. We do not use the "air puff" machine in our office. Your eyeglass prescription will be determined, and if you choose to wear contact lenses, the contact lens examination will be performed. We encourage all patients to opt-in for the dilated fundus examination of the back surfaces of the eye, as well as for the field of vision test, though these tests are optional and not covered by all vision insurance plans. Dilation causes light sensitivity, so please bring your sunglasses to your appointment if you plan to be dilated. If you have none we will provide a disposable pair to keep you protected. Upon completion of your exam the doctor will discuss her findings and review treatment options best suited for your needs and lifestyle.

After your exam you will meet with one of our skilled opticians in the dispensary for ordering and checkout. Our opticians are happy to assist you with eyewear decisions and insurance details. Most orders are delivered in 1-2 weeks, though we may be able to expedite your order for faster delivery.
